We’re here to provide excellent care to meet your schedule, whether it’s a planned visit or a need for treatment of an illness that came up suddenly.  No appointment is needed, but if you want to schedule a time to be seen, please call and speak with our office closest to you during business hours. Prior to coming to the clinic you may fill out the Patient Information Form, which includes insurance information, and the Patient History form.  Filling these out in advance will save some time at the clinic.

Insurance/Hospital Information

• Regular office co-pay - visits qualify for regular office co-pay, not emergency room co-pay rates.
• Cash Discount - For patients who have health insurance with a carrier with which we do not have a contract, a cash discount is available.
• x-rays are read by a group of radiologists, who will bill the patient directly.
• Accepted payment  - Visa, MasterCard, American Express, debit cards, cash and personal checks.  If you have questions, feel free to contact our office.
